Substitutionsbehandling av östrogenbristsymtom till kvinnor efter menopaus.
-
Förebyggande av osteoporos hos postmenopausala kvinnor med hög risk för framtida fraktur, om de inte tål eller har kontraindikationer mot andra läkemedel godkända för att förebygga osteoporos (se även avsnitt Varningar och försiktighet).
Behandlingen är avsedd för kvinnor med minst 6 månader sedan senaste menstruation. Erfarenheten av behandling av kvinnor över 65 år är begränsad.
Kontraindikationer

Dosering
Sequidot är en kontinuerlig sekventiell beredning för transdermal användning. En behandlingscykel består av 4 plåster Fas I följt av 4 plåster Fas II.
Behandlingen påbörjas med Fas I plåstret. Nästa behandlingscykel påbörjas omedelbart efter att det sista plåstret Fas II avlägsnats.
Vid behandlingsstart och vid fortsatt behandling av postmenopausala symtom ska lägsta effektiva dos användas under kortast möjliga tid (se även avsnitt Varningar och försiktighet).
Terapiinsättning
Behandlingen kan sättas in närhelst det passar till de flesta kvinnor efter menopaus, som inte får annan östrogenbehandling.
Kvinnor som redan står på kontinuerlig kombinerad östrogen/gestagen-terapi kan övergå till Sequidot direkt.
Kvinnor som står på cyklisk eller sekventiell östrogen/gestagen-terapi ska avsluta den pågående behandlingscykeln innan behandling med Sequidot Fas I påbörjas. Första dagen av en bortfallsblödning är lämplig tidpunkt att påbörja behandling med Sequidot.
Allmänna instruktioner
Fas I plåster appliceras på buken var 3:e eller var 4:e dag under de första 14 dagarna av en 28 dagars cykel. Därefter appliceras Fas II plåster på buken var 3:e eller var 4:e dag under de återstående 14 dagarna i cykeln.
Kvinnan bör upplysas om att en månatlig blödning vanligtvis förekommer.
Administreringssätt
Plåstret bör appliceras på buken och får aldrig appliceras på eller i närheten av brösten.
Plåstret fästs omsorgsfullt på en ren och torr hudyta som inte är skadad eller irriterad. Hudytan får inte vara fet dvs. plåstret ska inte användas tillsammans med fuktkräm, hudlotion eller hudolja. Midjelinjen bör undvikas eftersom tätt åtsittande klädesplagg kan skava av plåstret.
Plåstren får inte appliceras två gånger i följd på samma ställe. Det bör gå minst 1 vecka innan samma hudyta används igen.
Efter att ett kuvert med plåster öppnats, avlägsnas halva skyddsfilmen utan att sidan med klister vidrörs med fingrarna. Plåstret fästs omedelbart på hudytan. Därefter avlägsnas den andra halvan av skyddsfilmen och plåstret pressas fast genom att trycka med handflatan i minst 10 sekunder. Avslutningsvis pressas kanterna ned omsorgsfullt.
Försiktighet bör iakttas vid bad eller andra aktiviteter så att plåstret inte lossnar.
Om plåstret faller av (t.ex. efter ansträngande fysisk aktivitet, ymnig svettning eller friktion från tätt åtsittande klädesplagg) kan samma plåster appliceras igen på en annan hudyta. Det ursprungliga doseringsschemat följs därefter; dvs. bytesdagen ändras ej.
När plåstret applicerats på huden bör det inte utsättas för solljus under någon längre tid.
Har kvinnan glömt att applicera ett plåster ska ett nytt plåster appliceras så snart som möjligt. Fortsätt sedan att följa det ursprungliga schemat. Den avbrutna behandlingen kan orsaka att postmenopausala symtom, genombrottsblödningar och spotting uppträder.
Om häftmassa skulle finnas kvar på huden efter att plåstret avlägsnats bör man gnugga området försiktigt med en oljebaserad hudkräm eller lotion för att ta bort rester av häftmassan.
Varningar och försiktighet
För behandling av postmenopausala symtom ska HRT endast påbörjas om symtomen påverkar livskvaliteten negativt. Vid all behandling ska en noggrann värdering av risk/nytta-balansen göras minst en gång om året och HRT ska endast fortsätta så länge nyttan överväger riskerna.
Kunskap kring riskerna associerade med HRT i behandling av prematur menopaus är begränsad. På grund av låg absolut risk hos yngre kvinnor, kan dock nytta/risk-balansen för dessa kvinnor vara mer fördelaktig än för äldre kvinnor.
Medicinsk undersökning/uppföljning av behandling
Innan HRT inleds eller återupptas ska en noggrann anamnes tas, inklusive uppgifter om ärftliga sjukdomar. En allmän medicinsk och gynekologisk undersökning, som också inkluderar undersökning av brösten, ska göras med hänsyn tagen till patientens egen sjukhistoria och till avsnitt Kontraindikationer Kontraindikationer och 4.4 Varningar och försiktighet.
Under behandlingstiden rekommenderas regelbundna kontroller vars frekvens och utformning bör anpassas till den enskilda kvinnan. Kvinnan ska informeras om vilken typ av förändringar i brösten hon bör rapportera till sin läkare eller sjuksköterska/barnmorska (se avsnittet ”Bröstcancer” nedan).
Kontroller, inklusive regelbunden undersökning av brösten och/eller mammografi, ska utföras i enlighet med gällande rutiner för screening samt i övrigt anpassas efter den enskilda kvinnans kliniska behov.
Tillstånd som kräver skärpt uppmärksamhet
Vid förekomst av något av nedan angivna tillstånd eller om patienten tidigare haft tillståndet och/eller om det förvärrats under graviditet eller tidigare hormonbehandling, ska patienten övervakas speciellt. Hänsyn ska tas till att dessa tillstånd kan återkomma eller förvärras vid behandling med Sequidot:

Riskfaktorer för östrogenberoende tumörer, t.ex. första gradens ärftlighet för bröstcancer

Behandlingen bör avbrytas vid uppträdande av kontraindikationer (se avsnitt Kontraindikationer) samt i följande situationer:

Signifikant ökning av blodtrycket

Debut av migränliknande huvudvärk

Graviditet
Endometriehyperplasi och carcinom
För kvinnor med intakt livmoder är risken för endometriehyperplasi och endometriecancer ökad när enbart östrogen ges under lång tid. Den rapporterade ökningen av risk för endometriecancer hos kvinnor behandlade med enbart östrogen varierar mellan en fördubblad till 12 gånger större risk i jämförelse med icke-behandlade, beroende på behandlingens längd och östrogendos (se även avsnitt Biverkningar). Efter avslutad behandling kan risken förbli förhöjd i minst 10 år.
Tillägg av ett gestagen cykliskt under minst 12 dagar per månad/28 dagars behandlingscykel, eller kontinuerlig behandling med kombinerat östrogen-gestagen av icke-hysterektomerade kvinnor, minskar den ökade risken associerad med behandling med enbart östrogen.
Genombrottsblödning och/eller stänkblödning kan förekomma under de första behandlingsmånaderna. Om genombrottsblödning eller stänkblödning uppträder efter en viss tids behandling eller fortsätter efter avslutad behandling, ska orsaken utredas, vilket kan inkludera endometriebiopsi för att utesluta endometriemalignitet.
Bröstcancer
Den samlade kunskapen visar att det finns en ökad risk för bröstcancer hos kvinnor som använder HRT med en kombination av östrogen och gestagen eller med enbart östrogen. Risken är beroende av behandlingstidens längd.
Behandling med kombination av östrogen-gestagen

Den randomiserade placebokontrollerade studien, Women´s Health Initiative study (WHI), och en metaanalys av prospektiva epidemiologiska studier påvisar konsekvent ökad risk för bröstcancer hos kvinnor som behandlas med östrogen-gestagen i kombination som HRT som blir påtaglig efter ca 3 (1-4) år (se avsnitt Biverkningar).
Behandling med enbart östrogen
-
WHI-studien fann ingen ökad risk för bröstcancer hos kvinnor som genomgått hysterektomi och som behandlas med enbart östrogen. Observationsstudier har mestadels rapporterat en liten ökad risk för bröstcancer som är lägre än risken som hittats för östrogen-gestagen-kombinationer (se avsnitt Biverkningar).
Resultat från en stor metaanalys visade att den ökade risken minskar med tiden efter avslutad behandling, och att den tid det tar för att återgå till baslinjevärdena beror på hur länge den tidigare HRT-behandlingen har varat. Om HRT tagits i mer än 5 år kan risken kvarstå i 10 år eller mer.
HRT, speciellt kombinationer av östrogen och gestagen, ökar densiteten i mammografiska bilder. Detta kan försvåra möjligheten att radiologiskt upptäcka bröstcancer.
Ovarialcancer (Äggstockscancer)
Ovarialcancer är mycket mer sällsynt än bröstcancer.
Hos kvinnor som tar HRT med enbart östrogen eller kombinerat östrogen-gestagen, finns enligt epidemiologiska belägg från en stor metaanalys, en lätt förhöjd risk. Risken blir tydlig inom 5 års användning och går tillbaka med tiden efter avbruten behandling. Enligt andra studier, såsom WHI-prövningen, kan användning av kombinerade HRT‑preparat vara förknippat med en liknande eller något lägre risk (se avsnitt Biverkningar).
Venös tromboembolisk sjukdom
-
HRT är associerat med en 1,3 – 3 gånger större risk för utveckling av venös tromboembolism (VTE), dvs. djup ventrombos eller lungemboli. Förekomsten av en sådan händelse är mer trolig under det första året av HRT än senare (se avsnitt Biverkningar).

Allmänt erkända riskfaktorer för VTE inkluderar användning av östrogener, högre ålder, stora kirurgiska ingrepp, långvarig immobilisering, fetma (BMI > 30 kg/m2), graviditet och postpartum-perioden, systemisk lupus erythematosus (SLE) och cancer. Det råder ingen konsensus om den möjliga rollen för åderbråck i samband med VTE.

Patienter med kända trombofila tillstånd har en ökad risk för VTE och HRT kan öka denna risk. HRT är därför kontraindicerat för dessa patienter (se avsnitt Kontraindikationer).
-
Balansen mellan risk och nytta bör noga övervägas inför HRT till kvinnor som kroniskt behandlas med antikoagulantia.
-
Som hos alla postoperativa patienter bör förebyggande åtgärder övervägas för att förhindra VTE efter kirurgi. Om längre tids immobilisering kan förväntas efter en planerad operation rekommenderas uppehåll i substitutionsbehandlingen 4 – 6 veckor innan ingreppet. Behandlingen ska inte återupptas förrän kvinnan är fullständigt mobiliserad.

Kvinnor utan egen anamnes på VTE, men med en förstahandssläkting med historik av trombos i ung ålder, kan erbjudas utredning efter noggrann rådgivning angående dess begränsningar (endast en del av trombofila defekter identifieras av en utredning). Om en trombofil defekt identifieras som en annan typ än trombos hos familjemedlemmar eller om defekten har en ’ökad svårighetsgrad’ (t.ex. defekter för antitrombin, protein S eller protein C, eller en kombination av defekter) så är HRT kontraindicerat.

Om VTE utvecklas efter behandlingen påbörjats, bör preparatet sättas ut. Patienter ska uppmanas att omedelbart kontakta läkare vid symtom som kan tyda på VTE (t.ex. vid smärtsam svullnad av ett ben, plötslig bröstsmärta, dyspné).
Kranskärlssjukdom
-
Randomiserade kontrollerade studier har inte kunnat påvisa något skydd mot hjärtinfarkt hos kvinnor med eller utan befintlig kranskärlssjukdom som behandlats med kombinerat östrogen-gestagen eller enbart östrogen HRT.
Kombinerad östrogen-gestagen behandling
Den relativa risken för kranskärlssjukdom under behandling med kombinerat östrogen‑gestagen HRT är något ökad. Eftersom baslinjen för absolut risk för kranskärlssjukdom är starkt kopplat till ålder, är antalet extra fall av kranskärlssjukdom på grund av användning av östrogen‑gestagen, väldigt lågt hos friska kvinnor nära menopaus, men ökar med stigande ålder.
Behandling med enbart östrogen
Randomiserade kontrollerade data fann ingen ökad risk för kranskärlssjukdom hos kvinnor som genomgått hysterektomi och som behandlas med enbart östrogen
Ischemisk stroke
-
Behandling med kombinerad östrogen-gestagen och med enbart östrogen, är associerat med upp till 1,5 gånger ökad risk för ischemisk stroke. Den relativa risken förändras inte med ålder eller tidsintervall efter menopaus. Dock ökar den generella risken för stroke med åldern hos kvinnor som behandlas med HRT, eftersom baslinjen för stroke-risk är starkt åldersberoende (se avsnitt Biverkningar).
Hypotyreoidism
-
Patienter som behöver substitutionsbehandling med tyreoideahormon bör regelbundet kontrolleras avseende tyreoidafunktion när de behandlas med HRT för att säkerställa att tyreoideahormonnivåerna ligger kvar inom ett acceptabelt intervall.
Svåra anafylaktiska/anafylaktoida reaktioner

Fall av anafylaktiska/anafylaktoida reaktioner, som utvecklats när som helst under estradiolbehandling och krävde akut medicinsk behandling, har rapporterats efter marknadsgodkännandet.
Andra tillstånd
-
Östrogener kan ge vätskeretention varför patienter med hjärtsjukdom eller nedsatt njurfunktion bör observeras noga.
-
Kvinnor med känd hypertriglyceridemi bör noggrant följas upp under behandling med HRT eftersom sällsynta fall av starkt förhöjda triglyceridnivåer i plasma, som kan leda till pankreatit, har beskrivits vid östrogenbehandling till kvinnor med detta tillstånd.
-
Östrogener ökar mängden tyroideabindande globulin (TBG) vilket medför ökade nivåer av cirkulerande tyroideahormon, mätt såsom proteinbundet jod (PBI), T4-nivåer (mätt med kolonn eller med radioimmunoassay, RIA) och T3-nivåer (mätt med RIA). T3-resinupptaget minskar, vilket speglar de ökade nivåerna av TBG. Koncentrationerna av fritt T4 och fritt T3 är opåverkade. Även andra bindande proteiner kan öka i serum, t.ex. kortikosteroidbindande globulin (CBG) och könshormonbindande globulin (sex hormone binding globulin, SHBG), vilket leder till ökade nivåer av cirkulerande kortikosteroider respektive könssteroider. De fria eller biologiskt aktiva hormonkoncentrationerna förändras dock inte. Andra plasmaproteiner kan öka (substrat för angiotensin/renin, alfa-1-antitrypsin, ceruloplasmin).

Användning av HRT förbättrar inte kognitiv funktion. Det finns vissa bevis för en ökad risk för trolig demens hos kvinnor som börjar använda kontinuerlig kombinerad eller enbart östrogen HRT efter 65 års ålder.
-
Kontaktsensibilisering kan förekomma vid alla topikala applikationer. Även om det är mycket sällsynt kan kvinnor utveckla kontaktsensibilisering mot något av innehållsämnena i plåstret och ska varnas för att svår hypersensibilisering kan inträffa vid fortsatt exponering för det orsakande ämnet.

Exogena östrogener kan orsaka eller förvärra symtomen på ärftligt eller förvärvat angioödem.
ALAT-förhöjningar
Under kliniska studier med hepatit C-virus (HCV)-kombinationsbehandlingen ombitasvir/paritaprevir/ritonavir med eller utan dasabuvir, var ALAT-förhöjningar på mer än 5 gånger den övre normalvärdesgränsen signifikant mer frekvent förekommande hos kvinnor som använde läkemedel innehållande etinylestradiol, så som kombinerade hormonella preventivmedel. Dessutom observerades ALAT-förhöjningar även hos kvinnor som behandlades med glekaprevir/pibrentasvir och som använde läkemedel innehållande etinylestradiol, t.ex. kombinerade hormonella preventivmedel. Kvinnor som använde läkemedel innehållande andra östrogener än etinylestradiol, så som estradiol, hade en ALAT-förhöjning liknande de som inte fått några östrogener; men på grund av det begränsade antalet kvinnor som tar dessa andra östrogener bör försiktighet iakttas vid samtidig administrering med kombinationsbehandlingen ombitasvir/paritaprevir/ritonavir med eller utan dasabuvir och även behandlingen glekaprevir/pibrentasvir. Se avsnitt Interaktioner.
Interaktioner
Metabolismen av östrogener och gestagener kan öka vid samtidig behandling med substanser som är kända för att inducera enzym som metaboliserar läkemedel, speciellt cytokrom P450 enzymer. Exempel på sådana substanser är antiepileptika (t.ex. fenobarbital, fenytoin, karbamazepin) och vissa medel mot infektioner (t.ex. rifampicin, rifabutin, nevirapin, efavirenz).
Trots att ritonavir, telaprevir och nelfinavir är kända som starka hämmare av läkemedelsmetaboliserande enzym, har dessa substanser, när de ges tillsammans med steroidhormoner, inducerande egenskaper.
Naturläkemedel innehållande Johannesört (Hypericum perforatum) kan också inducera metabolismen av östrogener och gestagener.
Estradiol metaboliseras huvudsakligen av CYP3A4, varför samtidig administrering med CYP3A4-hämmare såsom ketokonazol, erytromycin, kan medföra en ökad exponering av estradiol.
Vid transdermal tillförsel undviks första passage-effekt i levern och därför kan transdermalt tillfört östrogen och gestagen antas påverkas i mindre utsträckning än peroralt intagna hormoner av andra enzyminducerande substanser.
Den kliniska betydelsen av en ökad metabolism av östrogener och gestagener kan vara minskad effekt och förändringar i den uterina blödningsprofilen.
Vissa blodprovsanalyser kan påverkas av östrogenbehandling, t.ex. glukostoleranstest och sköldkörtelfunktionstest.
Farmakodynamiska interaktioner
Under kliniska studier med HCV-kombinationsbehandlingen ombitasvir/paritaprevir/ritonavir med eller utan dasabuvir, var ALAT-förhöjningar på mer än 5 gånger den övre normalvärdesgränsen signifikant mer frekvent förekommande hos kvinnor som använde läkemedel innehållande etinylestradiol, så som kombinerade hormonella preventivmedel. Kvinnor som använde läkemedel innehållande andra östrogener än etinylestradiol, så som estradiol, hade en ALAT-förhöjning liknande de som inte fått några östrogener; men på grund av det begränsade antalet kvinnor som tar dessa andra östrogener bör försiktighet iakttas vid samtidig administrering med kombinationsbehandlingen ombitasvir/paritaprevir/ritonavir med eller utan dasabuvir och även behandlingen glekaprevir/pibrentasvir (se avsnitt Varningar och försiktighet).
Graviditet
Sequidot är inte indicerat under graviditet. Om graviditet inträffar under behandling, ska behandlingen avbrytas omgående.
Kliniska data från ett begränsat antal graviditeter som exponerats för noretisteronacetat, tyder inte på några fosterskadande effekter. Vid doser högre än vad som normalt används i perorala konceptionsmedel och HRT-beredningar har maskulinisering av kvinnliga foster observerats.
Resultaten från de flesta epidemiologiska studier som genomförts hittills och som är relevanta gällande oavsiktlig fetal exponering med kombinationer av östrogen och gestagen, tyder inte på teratogena eller fetotoxiska effekter.
Amning
Sequidot är inte indicerat under amning.
Trafik
Inga kända effekter på förmågan att framföra fordon och använda maskiner.
Biverkningar
Ungefär en tredjedel av alla kvinnor behandlade med Sequidot kan förvänta sig biverkningar. Vanligast rapporterade biverkan är bröstspänningar och smärta i brösten (31 %), reaktioner på applikationsstället (20 % vanligen lindrigt erytem), dysmenorré (19 %), oregelbundna blödningar (16 %) och huvudvärk (10 %).

Risk för bröstcancer

En upp till dubblerad risk för att få diagnosen bröstcancer har rapporterats för kvinnor som fått kombinerad behandling med östrogen och gestagen i mer än 5 år.
-
Den ökade risken för kvinnor som använder enbart östrogen är lägre än för kvinnor som använder en kombination av östrogen och gestagen.
-
Risken är beroende av behandlingstidens längd (se avsnitt Varningar och försiktighet).
-
Beräkning av absolut risk baserad på resultat från den största randomiserade placebokontrollerade studien (WHI-studien) och den största metaanalysen av prospektiva epidemiologiska studier presenteras nedan.

Risken för endometriecancer
Postmenopausala kvinnor med kvarvarande livmoder
Risken för endometriecancer är cirka 5 fall per 1 000 kvinnor med kvarvarande livmoder som inte använder HRT.
För kvinnor med kvarvarande livmoder rekommenderas inte användning av enbart östrogen HRT eftersom det ökar risken för endometriecancer (se avsnitt Varningar och försiktighet).
Beroende på behandlingstidens längd och dosen östrogen, varierar riskökningen för endometriecancer i epidemiologiska studier mellan 5 och 55 extra fall per 1 000 kvinnor i åldern mellan 50 och 65 år.
Tillägg av en gestagen till östrogen-behandlingen i åtminstone 12 dagar per cykel kan förebygga denna ökade risk. I studien ’Million Women Study’ (MWS) visade fem års kombinerad HRT (sekventiell eller kontinuerlig) ingen ökad risk för endometriecancer (Relativ Risk på 1,0 (0,8-1,2)).
Ovarialcancer (Äggstockscancer)
Användning av HRT med enbart östrogen eller kombinerat östrogen-gestagen har förknippats med en lätt förhöjd risk för att få diagnosen ovarialcancer (se avsnitt Varningar och försiktighet).
Vid en metaanalys från 52 epidemiologiska studier rapporterades en förhöjd risk för ovarialcancer hos kvinnor som använder HRT jämfört med kvinnor som aldrig använt HRT (RR 1,43; 95-procentigt KI 1,31-1,56). För kvinnor i åldern 50 till 54 år som tar HRT i 5 år ger detta omkring 1 extra fall per 2000 användare.
För kvinnor i åldern 50 till 54 som inte tar HRT kommer ungefär 2 av 2000 kvinnor diagnosticeras med ovarialcancer under en 5-årsperiod.
Risk för venös tromboembolism
HRT är associerat med en 1,3-3 gånger större relativ risk för att utveckla venös tromboembolism (VTE), dvs. djup ventrombos eller lungemboli. Förekomsten av en sådan händelse är mer trolig under det första året av HRT än senare (se avsnitt Biverkningar). Resultat från WHI-studier presenteras nedan:

Risk för kranskärlssjukdom

Risken för kranskärlssjukdom är något förhöjd hos användare av kombinerat östrogen-gestagen HRT över 60 års ålder (se avsnitt Varningar och försiktighet).
Risk för ischemisk stroke

Behandling med enbart östrogen och kombinerad östrogen-gestagen är associerat med upp till 1,5 gånger ökad relativ risk för ischemisk stroke. Risken för haemorragisk stroke är inte ökad under användning av HRT.

Denna relativa risk är inte beroende av ålder eller behandlingstidens längd, men eftersom baslinjerisken är starkt beroende av ålder, kommer den totala risken för stroke hos kvinnor som använder HRT att öka med åldern (se avsnitt Varningar och försiktighet).

Överdosering
På grund av administreringssättet är det osannolikt att överdosering av estradiol eller noretisteron ska inträffa. Om tecken på överdosering uppkommer ska depotplåstret avlägsnas från huden. Symtom på överdosering vid oral behandling med östrogener är bröstspänningar, illamående, kräkning och/eller metrorragi. Överdosering av gestagener kan leda till nedstämdhet, trötthet, akne och hirsutism.
Farmakodynamik
Den aktiva substansen, estradiolhemihydrat, syntetiskt 17β-estradiol, är kemiskt och biologiskt identisk med endogent, humant estradiol. Den ersätter den förlorade östrogenproduktionen hos kvinnor efter menopaus och lindrar menopausala symtom.
Östrogener förhindrar benförlust efter menopaus eller efter ooforektomi.
Eftersom östrogen stimulerar tillväxten av endometriet ökar risken för endometriehyperplasi och endometriecancer om det ges ensamt. Tillägg av noretisteronacetat, ett gestagen, reducerar den östrogeninducerade risken för endometriehyperplasi hos kvinnor som inte är hysterektomerade.
Information från kliniska prövningar

Lindring av symtom på östrogenbrist och information om blödningsmönster.
Lindring av menopausala symtom uppnåddes under behandlingens första veckor. Efter 11 behandlingscykler med Sequidot fick 64% av kvinnorna regelbundna bortfallsblödningar. Oregelbunden blödning och/eller stänkblödning rapporterades hos 28% och amenorré hos 8%.

Osteoporosprofylax.
-
Östrogenbrist efter menopaus är associerat med en ökad benomsättning och en minskning av benmassan. Effekten av östrogen på benmineralinnehållet (Bone Mineral Density, BMD) är dosberoende. Effekten tycks kvarstå så länge behandlingen pågår. Efter avslutad HRT sker förlusten av benmassa över tid i ungefär samma takt som hos obehandlade kvinnor.

Resultat från WHI-studien och från meta-analys av andra studier visar att HRT med enbart östrogen eller med östrogen-gestagen i kombination, givet till företrädesvis friska kvinnor, minskar risken för höft- och kotfrakturer och andra osteoporosfrakturer. HRT kan även förhindra frakturer hos kvinnor med låg benmassa och/eller med diagnostiserad osteoporos. Bevisen för detta är dock begränsade.

Efter två års behandling med Sequidot hade BMD i ländryggen ökat med 5,53 %±0,63 % (medelvärde±SD). 95,0 % av kvinnorna behöll eller ökade sin BMD i ländryggen under behandlingen.

Sequidot hade också effekter på BMD i höften. Ökningen efter två år var 3,07 %±0,64 % (medelvärde±SD) i lårbenshalsen och 3,12 %±0,46 % (medelvärde±SD) i hela höften.

Farmakokinetik
Absorption
Transdermalt administrerat estradiol undgår den första passage-effekt som ses med oralt administrerade östrogenprodukter.
Estradiol: Sequidot depotplåster ger serumnivåer av estradiol och estron/estradiolkvoter inom det område som normalt ses hos premenopausala kvinnor i tidig (estradiol > 40 pg/ml) till intermediär follikelfas. Dessa karaktäristika upprätthålls under en hel användningsperiod, 84-96 timmar. Upprepad applicering av Estalis Sekvens fas I (50/0 mikrogram/dag) depotplåster, som är bioekvivalent till Sequidot fas I, resulterade i maximala estradiolkoncentrationer i serum (Cmax) vid steady-state på 71 pg/ml och genomsnittlig estradiolkoncentration i serum (Cmedel) vid steady-state på 51 pg/ml. Vid appliceringsintervallets slut var den genomsnittliga estradiolkoncentrationen i serum (dalvärdet) 41 pg/ml.
Upprepad applicering av Sequidot fas II (50/250 mikrogram/dag) depotplåster resulterade i maximala estradiolkoncentrationer i serum (Cmax) vid steady-state på 71 pg/ml och genomsnittlig estradiolkoncentration i serum (Cmedel) vid steady-state på 52 pg/ml. Vid appliceringsintervallets slut var den genomsnittliga estradiolkoncentrationen i serum (dalvärdet) 46 pg/ml.
Noretisteronacetat: Upprepad applicering av Sequidot resulterade maximala noretisteronkoncentrationer i serum (Cmax) vid steady-state på 1060 pg/ml och genomsnittlig noretisteronkoncentration i serum (Cmedel) vid steady-state på 832 pg/ml. Vid appliceringsintervallets slut var den genomsnittliga noretisteronkoncentrationen i serum (dalvärdet) 681 pg/ml.
Metabolism och eliminering
Estradiol: Estradiol har en kort eliminations-halveringstid på ca 2-3 timmar varför serumnivåerna snabbt avklingar när depotplåstret avlägsnats. Efter att plåstret tagits bort återgår serumkoncentrationerna av estradiol till obehandlade postmenopausala nivåer (< 20 pg/ml) inom 4-8 timmar.
Noretisteron: Eliminationshalveringstiden för noretisteron har rapporterats vara ca 6-8 timmar. Efter att plåstret avlägsnats minskar serumkoncentrationen av noretisteron snabbt och är mindre än 50 pg/ml inom 48 timmar.
Minimala fluktuationer i serumkoncentrationer av estradiol och noretisteron visar att utsöndringen är konstant över hela doseringsintervallet.
Ingen ackumulering av estradiol eller noretisteron sker efter upprepad applicering.
Prekliniska uppgifter
Toxicitetsprofilerna för estradiol och noretisteron är väl kända. Långvarig kontinuerlig tillförsel av naturliga och syntetiska östrogener i vissa djurarter ökar frekvensen av karcinom i brösten, livmoder, livmoderhals, vagina, testiklar och lever. Långvarig kontinuerlig tillförsel av noretisteron i vissa djurarter ökar frekvensen av tumörer i hypofys och äggstockar hos kvinnor och i lever och bröst hos män.

1. 17β-estradiol and its main metabolites estrone and estriol
Environmental risk: Use of 17β-estradiol has been considered to result in a moderate environmental risk. Both 17β-estradiol and its two main metabolites estrone and estriol are considered.
Degradation: 17β-estradiol is slowly degraded in the environment.
Bioaccumulation: 17β-estradiol is assessed not to have a high potential for bioaccumulation. The two main metabolites, estrone and estriol are considered to have a low poten-tial for bioaccumulation.
PBT/vPvB: Neither 17β-estradiol nor its two main metabolites are considered to be PBT/vPvB substances.
Detailed background information
2. The active pharmaceutical ingredients (API)
17β-estradiol is used for hormone replacement therapy of women with menopause complications.
17β-estradiol is metabolized during human metabolism into the major transformation products estrone, estriol, estrone sulfate and estrone glucoronide (Ref. 31, 48, 65).
17β-estradiol, estrone and estriol are natural estrogens which belong to the class of steroid hormones. 17β-estradiol is the primary female sex hormone and estrone is the primary metabolite of 17β-estradiol.

3. Environmental Risk classification (PEC/PNEC ratio)
3.1 Predicted Environmental Concentration (PEC)
PEC (Predicted Environmental Concentration) is calculated according to the following formula:

A = Total amount of API (kg) sold in Sweden in a given year. The total amount of estradiol (hemihydrate and valerat) sold in Sweden in 2020 was 20.86 kg API based on IQVIA/LIF sales data (Ref. 10). Reduction of A may be justified based on metabolism data. It can be assumed that 17β-estradiol is metabolised in the female body and excreted as 33% 17β-estradiol, 54% Estrone and 13% Estriol (Ref. 5), so A is set to:

R = Removal rate (%) due to loss by adsorption to sludge particles, by volatilization, hydrolysis or biodegradation. R = 0 if no data is available. The removal rates are based on estimation of distribution of estrogens in a municipal wastewater treatment plant in accordance with the principles of the EU TGD (Ref. 10), and by use of the program SimpleTreat 3.0, which estimates the relative distribution of chemicals to each compartment: effluent, sludge and air. The following removal rates (R) in wastewater treatment plants are estimated (Ref. 5):

17β-estradiol: 40% ; Conjugated 17β-estradiol: 6-8%. 17β-estradiol is excreted by mammals as glucuronide or sulfate conjugates in urine or in the unmetabolized form in faeces. Adler et al. (Ref. 12) reported that 50% of 17β-estradiol and 58% of estrone were conjugated in raw sewage. Furthermore, they found by measurement that 87% of the non-conjungated 17β-estradiol was removed in wastwater treatment plant and 47% of the conjungated 17β-estradiol was removed. Overall, a measured removal of 67% was found for 17β-estradiol and its conjugates. Thus, it is considered conservative to keep the SimpleTreat estimated removal for 17β-estradiol of 40%.

Estrone: 8%; conjugated estrone: 0%. Adler et al. (Ref. 12) measured that 55% of the estrone was removed whereas a slightly higher concentration of the conjugated in the effluent than in the effluent was found (approximately 7.5 ng/L conjugate in the inlet and 8 ng/L conjugate in the outlet). Overall, a measured removal of 19% was found for estrone and its conjugates. Thus, it is considered conservative to keep the SimpleTreat estimated removal for estrone of 8%.

3.2 Predicted No Effect Concentration (PNEC)
Available eco-toxicological data for 17β-estradiol, estrone and estriol and the derivation of PNEC-values is presented in this section.
3.2.1 17β-estradiol
A proposed EU EQS (PNEC) value has been derived for the 17β-estradiol (Ref. 7) in connection with setting 17β-estradiol on a short-list of 19 possible new priority substances for the Water Frame Directive (Ref. 6). The data used for the derivation of the EQS-value is presented in Appendix together with the derivation, and only a short overview of the derivation is given here.
Knowledge of the mode of action of 17β-estradiol - and strongly supported by the acute and chronic test toxicity data (see Appendix) - suggests that fish and amphibians are likely to be the most sensitive organisms. This is supported by the available chronic toxicity data which indicates that fish are particularly sensitive to 17β-estradiol. Two studies were located on amphibians with LOECs in the range of 1000-2740 ng/l reported for Rana pipens and Xenopus laevis. These LOECs are far above the NOECs for fish. Therefore, a SSD (Species Sensitivity Distribution) was derived for 17β-estradiol based on data for the most sensitive taxonomic groups, fish - expecting that chronic fish data used for the derivation of an SSD would also be protective of the other less sensitive group.
The lowest no observed effect concentration for 17β-estradiol is a 35-50 d NOEC of 0.5 ng/l (Ref. 48) for the trout (Onchorhynchus mykiss). The observed effects were sperm volume, sperm density and fertilization success. The study was not carried out according to a guideline. Experiments took place in four identical flow-through 0.5 m3 tanks (three replicates and one control - each tank with 10 males and 3 females of approximate same size). Water inflow temperature was 6oC and air saturation of water was >90%. Fish were kept under natural photoperiod (experiments were carried out in Kreuzstein in Sankt Gilgen, Upper Austria during December – January).
Overall, reliable chronic NOEC values were available for 11 species of fish and the SSD was based on these 11 fish species (Ref. 7). The HC5 for the SSD was found at 0.8 ng/l. Based on the available dataset and the knowledge of the mode of action, an assessment factor of 2 was considered appropriate. This gives an AA-EQS of 0.4 ng/l.
This derivation of the AA-EQS was reviewed by SCHER (Ref. 8). Both the reliability and the ecological relevance of the endpoints and taxonomic groups were considered. Overall, the SCHER supported the proposed AA-EQS of 0.4 ng/l for 17β-estradiol.
In conclusion, a PNEC of 0.4 ng/L is used for 17β-estradiol
3.2.2 Estrone
A PNEC-value has been derived for estrone in connection with setting the substance (together with 17β-estradiol) on a short-list of 19 possible new priority substances for the Water Frame Directive (Ref. 6).
A well-accepted EU PNEC for estrone has been derived at 3.6 ng/l (Ref. 59).
Environmental toxicity data for estrone has been collected and are presented in the annex.
As for 17β-estradiol, the mode of action for estrone suggests that fish and amphibians are likely to be the most sensitive organisms. Based on available data, fish is found to be the most sensitive species to estrone. A NOEC for estrone of 36 ng/l was obtained in 40-day study with Danio rerio (according to OECD Draft Test Guideline: A 40-day Juvenile Zebrafish Assay for screening of Endocrine Disrupting Chemicals), and a NOEC for estrone of 5 ng/l was obtained in a 90-day study (no guideline followed, fish specie: Oryzias latipes, effects measured: Organ weight in relationship to body weight; hatch, Vitellogenin 1 mRNA).
As for 17β-estradiol, the mode of action for estrone is well-known and fish is the most sensitive species. Therefore, an assessment factor of 10 for the chronic fish toxicity data is considered justified.
Using an assessment factor of 10, a PNEC of 0.5 ng/L was obtained.
3.2.3 Estriol
As for 17β-estradiol and estrone, the mode of action for estriol is well-known and fish is the most sensitive species. Therefore, an assessment factor of 10 for the chronic fish toxicity data is considered justified.
The No Observed Effect Concentration (NOEC) for induction of vitellogenin, which is considered a chronic eco-toxicity test, is found at 0.0465 µg/l for estriol (Ref. 49; not-a guideline study; test species Oryzias latipes, duration of study 90 days, temperature: 25 ± 1 °C, three replicates and one control; 30 embryos per replicate).
Using an assessment factor of 10, a PNEC of 4.7 ng/L was obtained.
3.2.4 Derived PNECs
PNEC for the three APIs in surface water is:

Based on the calculated PEC/PNEC ratios and information about degradation, bioaccumulation and eco-toxicity of 17β-estradiol, estrone and estriol the following environmental risk phrase should be applied to pharmaceutical products with estrogens according to the criteria in the FASS.se guidelines (Ref. 1):
”Use of pharmaceutical products with estrogens has been considered to result in moderate environmental risk”
This risk phrase is according to the FASS.se guidelines applicable for risk quotients in the interval: 1 < PEC/PNEC ≤ 10.
4. Biotic degradation
4.1. Degradation of 17β-estradiol
Activated sludge test according to OECD guideline no. 302A has shown that 17β-estradiol is inherently biodegradable under aerobic conditions in activated sludge (Ref. 30). 17β-estradiol is thus slowly degraded in the environment. In a 100 days simulation study of 17β-estradiol (OECD Test Method no. 308), an aerobic mineralisation (marine) of 61±1% respectively 62±3% mineralisation (freshwater) was found (Ref. 86). Thus, 17β-estradiol is found to be biodegradable in both marine and freshwater. In addition, an activated sludge tests (OECD 302, Ref. 2) show that 17β-estradiol is inherently biodegradable under aerobic conditions.

5. Bioaccumulation
According to the FASS.se guidelines (Ref. 1), substances with Log Pow ≥ 4 or BCF ≥ 500 are considered to have high potential for bioaccumulation. Valid BCF-data has prevalence above log Pow data. One limitation in the use of log Pow for the estimation of the bioaccumulation potential is that metabolism within the test organism is not considered.

It is noted that 17β-estradiol has a calculated log Pow slightly below but close to the cut-off value of 4. It can be mentioned that a logPow slightly above 4 (4.01) has been measured (Ref. 33, method not reported). Several measured BCFs are available for 17β-estradiol – all well below the cut-off value of 500. Therefore, 17β-estradiol is assessed not to have a high potential for bioaccumulation.
Both estrone and estriol have calculated log Pow well below 4. Actually, measured log Pow values are available for the two substances showing a log Pow of 3.13 respectively 2.45 (Ref. 33, method not reported). In addition, a BCF well below 100 is measured for estrone in the fish “high-back crucian carp”. Thus, both substances are considered to have a low potential for bioaccumulation.
Of some interest to note is the measured partitioning between water and synthetic membrane liposomes – mimicking biological specie-of the three substances. The partitioning of 17β-estradiol and estrone is on the very same level – whereas the partitioning of estriol to the membrane liposomes is much lower. This is in agreement with the calculated log Pow-values.
Overall, it is assessed that 17β-estradiol, estrone and estriol all have a low potential for bioaccumulation.
6. PBT/vPvB assessment
Considering all three aspects, 17β-estradiol, estrone and estriol do not meet the criteria for classification as a PBT or vPvB substance.

Acute effects have been considered of no relevance and therefore no MAC-EQS has been derived.
Chronic toxicity data for 17β-estradiol is available for a range of species including algae, crustaceans, rotifers, amphibians and fish. It is concluded that the critical effect due to exposure of 17β-estradiol and its primary metabolites estrone and estriol is the induction of vitellogenin in fish that may cause a change in sex from male to female.
In order to apply the SSD (Species Sensitivity Distribution) approach the available dataset should preferably contain more than 15, but at least 10 NOECs/EC10s from different species covering at least 8 taxonomic groups. For estimating an AA-EQS freshwater using the SSD approach the following taxa would normally need to be represented, i.e.

The available chronic toxicity dataset for 17β-estradiol does not meet the data requirements for using the SSD approach. However, 17β-estradiol is a naturally occurring hormone and has a specific mode of action with effects on the reproductive physiology of vertebrates. The EU guidance notes that if a chemical is known to have a specific mode of action an SSD can be derived for only those taxa that are expected to be particularly sensitive.
Knowledge of the mode of action of 17β-estradiol suggests that fish and amphibians are likely to be the most sensitive organisms. This is supported by the available chronic toxicity data which indicates that fish are particularly sensitive to 17β-estradiol. Two studies were located on amphibians with LOECs in the range of 1000-2740ng/l reported for Rana pipens and Xenopus laevis. It is therefore proposed that an SSD is derived for β -estradiol based on data for the most sensitive taxonomic groups. It is expected that based on knowledge of the mode of action the chronic fish data the derivation of an SSD based on fish species only should be protective of other less sensitive group.
Reliable chronic NOEC values were available for 11 species of fish. An SSD has therefore been derived based on 11 fish species. For several species a number of different studies have been reported. The EU guidance on the derivation of an SSD indicates that where a number of data points are available for a species a geometric mean should be calculated to propose a single value for a species. This approach is not appropriate for all the available data as the studies are often non-standard and consider a range of endpoints and exposure durations and are therefore not directly comparable. In these cases, the lowest NOEC value is used for a species.
The SSD based on the fish data is shown below. The distribution fit to a log normal distribution.
The HC5 from the above SSD is 0.8 ng/l. An assessment factor in the range of 1-5 should be applied to the HC5 based on the guidance given in the TGD-EQS (E.C., 2011). Based on the available dataset and the knowledge of the mode of action it is considered that an assessment factor of 2 (mode of toxic action is well understood, HC5 has been derived based on data for the most sensitive taxonomic group, a wide range of endpoints and durations including population relevant endpoints such as hatching, fertilisation, changes in sex ratio are included in the dataset) is appropriate for the derivation of the AA-EQS.
This gives a EQS of 0.4 ng/l.
The derivation of the AA-EQS has been reviewed by SCHER (Ref. 8). Both the reliability and the ecological relevance of the endpoints and taxonomic groups have been considered. Overall, the SCHER supports the proposed AA-EQS of 0.4 ng/l.

[1]It was found that the Vtg gene in male medaka fish can be induced by estriol at environmentally relevant concentration of 5 ng/L. However, it was noted that the Vtg mRNA changes are hardly ever reflected in concomitant changes in functional protein. Therefore, further studies were concluded to be needed to detect more sex hormone pathway gene expressions and functional protein levels to evaluate comprehensively estrogen potency of estriol in fish.
Method#1: A Fish Sexual Development Test (FSDT) (an extension of the existing OECD TG 210, fish early life stage toxicity test).
Method#2: Measurement of the impact of estriol on the embryonic development, sex differentiation, growth, and changes of functional genes related to reproduction of medaka (O. latipes) exposed to different concentrations of estriol during embryo-larval-, juvenile- and adult life stages. The corresponding time to hatching, hatchability, gross abnormalities, sex ratio, hepatosomatic index (HSI), gonadosomatic index (GSI), and changes of Vtg-I and ERα genes in livers of the fish exposed to estriol for 90 days were determined. Embryos less than 4 h post-fertilization were used in the exposure experiments. The embryos were exposed to nominal estriol concentrations of 5, 50, 500 and 5000 ng/L in charcoal-dechlorinated tap water for 15 days. Each exposure level had 3 replicate test concentrations with 30 embryos per replicate. In addition, solvent controls (SC) were included in the experimental design. The embryos in each group were placed in a glass dish and incubated on a 16:8 h light: dark photoperiod cycle at 25 ± 1 °C. Eighty percent of the test solution was renewed every 24 h. Hatchability, time to hatching and gross abnormalities were recorded. Once hatched, the hatched fry were continuously maintained at the same concentrations for the additional 15 days. After the additional 15 days of exposure, the genetic sex ratio was determined. Ten fish including five females and five males were assigned randomly to a 5-L glass aquarium and duplicate aquaria were used at each exposure level. Fish were continuously exposed to nominal estriol concentrations of 5, 50, 500, and 5000 ng/L and the SC was included in the experiment design. The solution was renewed every 24 h. Treated and control fish were exposed for another 60 days. The entire test duration was 90 days.

1. Norethisterone acetate (NETA)
Environmental risk: A valid risk quotient (PEC/PNEC) for NETA cannot be calculated due to lack of eco-toxicity data. NETA is very toxic to green algae (Desmodesmus subspicatus).
Degradation: NETA is potentially persistent in the environment.
Bioaccumulation: NETA has low potential for bioaccumulation.
PBT/vPvB assessment: NETA does not meet the criteria for classification as a PBT or vPvB substance.
Since the PEC/PNEC cannot be calculated due to lack of eco-toxicity data the following environmental risk phrase should be applied to pharmaceutical products containing NETA according to the criteria in ref. 1:

Norethisterone acetate (NETA), also known as norethindrone acetate, is a steroidal progestin that is used as a hormonal contraceptive. It is an acetate ester of norethisterone which belongs to the class of steroid hormones.

According to ref. 1, calculation of PNEC (Predicted No Effect Concentration) in surface water should be based on eco-toxicological data for three trophic levels. However, it has only been possible to present eco-toxicological data for two trophic levels i.e. green algae and daphnia. Furthermore, it is not known if these organisms are the most sensitive to NETA.
Consequently, it is not possible to calculate a valid PNEC according to the requirement in ref. 1 on basis of the available eco-toxicological data.
